The form can be found at http://amadistrict16.com/wp-content/uploads/2018Application.pdf The 2018 pdf is an autofill style, so you can complete it before printing. Save it to your system, or just print a duplicate for your records. When you join or renew your National AMA membership this year, consider doing it online by going to http://www.amajoin.com/WIOBTR. Doing it this way WOTA gets a small part of the fee. |
