BOTH parents must be present to sign the release form at sign up. NO exceptions as per the AMA. You can fill out, have notorized and send the Annual Minor Release Form from the AMA website, and get the Annual Release card back to eliminate this hassle. With this card, you don't even have to fill out the Minor Release form at the event. Title: Re: URGENT! I will be bringing my grandson this year to compete. We are sending for the AMA release card, but if it doesn't get back in time for the first event, does the single event release form have a way for a single parent to state they have sole custody?

My first reaction would be most sign-up folks would take her at her word about being sole custodian. However, perhaps, to be perfectly clear, your stepdaughter could write up a statement. stating she has sole custody, have IT notarized, and the sign-up folks could staple it to the minor release...until the cardboard form arrives. In the case of a a minor with both parents but only one wants to attend, have the non-present parent sign a letter giving permission for the other to take the kid to the event ( name, place and date of the event), and have it notarized stating other parent is the complete guardian of the day.
